I hereby consent to the service terms and conditions stated below:

 

• That I am participating in virtual program offered by ONELIFE during which I will receive information about Pilates, Functional Training, and Musculo-Skeletal Focused Physical Therapy exercises via a video conferencing tool. I recognize that this program will require physical exertion   on my own, which may be strenuous and may cause physical injury,  and I am  fully aware of the  risks and hazards involved.

 

• I understand that it is my responsibility to consult with a physician prior to and regarding my participation in this virtual program. I represent and warrant that I am physically fit for this activity and that I have the valid and updated medical clearance to fully participate in ONELIFE's program.

 

• I understand that my virtual sessions will be recorded for purposes of service quality control and assurance. I understand that my session recordings will not be released for public access.
   

• I understand that my requested coaching staff cannot be guaranteed due to the changing availability of ONELIFE coaching staff. I consent to working with a team of different coaches throughout my program.

 

• I am aware that there is a level of risk associated with this activity and I assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the program.

 

• I promise not to incite or influence employee, co-worker, or any person working for or on behalf of ONELIFE to engage in, or participate, directly or indirectly, in any form, fashion, or manner, as partner, officer, director, stockholder, advisor, consultant, employee, independent contractor, agent, or in any other form or capacity, in any other business similar to the ONELIFE’s business or any allied trade or undertakings.

 

• I shall not engage an employee, co-worker, or any person working for or on behalf of ONELIFE in any personal or professional capacity outside the agreed scope of service with ONELIFE.

 

• I fully acknowledge that ONELIFE strictly prohibits harassment of any kind, including sexual or verbal harassment, and ONELIFE will take appropriate and immediate action in response to complaints or knowledge of violations of this policy.

 

• I promise not to harass, in any verbal or physical conduct designed to threaten, intimidate or coerce an employee, co-worker, or any person working for or on behalf of ONELIFE.

 

• In further consideration of being permitted to participate in the program, I (with my heirs and representatives) knowingly, voluntarily and expressly waive any claim that I may have against ONELIFE and its staff for any acts of negligence, injury, and damages that I may sustain as a result of participating in the program.

 

• I will be held legally liable for any violation of the above-mentioned conditions.
   

• I acknowledge that this document was fully explained to me in a language known to me before signing it, and that this document is made willingly and voluntarily and with full knowledge of my rights under the law.
